How to evaluate the caries activity in enamel and dentine with the ICDAS criteria?

Surface of enamel is whitish/yellowish opaque with loss of luster; feels rough when the tip of the probe is moved gently across the surface.

Lesion is in a plaque stagnation area, i.e.: pits and fissures, near the gingival and approximal surface below the contact point.

Surface of enamel is whitish, brownish or black. Enamel may beshiny and feels hard and smooth when the tip of the probe is moved gently across the surface.

For smooth surfaces, the caries lesion is typically located at some distance from the gingival margin.

Cavity feels soft or leathery on gently probing the dentin.

Cavity may be shiny and feels hard on gently probing the dentin.

 

 Active lesion

  • In the right image a code 3 of ICDAS, there is a buccall enamel defect smaller than 0.5 mm
  • There is thick  bacterial plaque present in the lesion
  • There is bleeding from the gum to the touch with the probe
  • Clean the area, to remove the bacterial plaque.
  • The lesion looks dull.
  • And the soft probing appears rough and hard.
  • The injury is active.
  • In the rigth image a code 1 (Brown spot) in dry.

  • No plaque present

  • IMPORTANT: Plaque no present, because the patient clean teehts before to consult.

  • There is bleeding from the gum to the touch with the probe
  • And the soft probing appears rough and hard.
  • Active lesion

Arrested lesion

  • In the right image a code 2 of ICDAS,  in wet and dry.
  • No plaque present
  • There is not bleeding from the gum to the touch with the probe
  • And the soft probing appears soft and hard.
  • Arrested lesion
